- Send your full name, birthday, contact number, picture of your HMO card, valid ID, and doctor’s request with diagnosis.
- Once your LOA is approved, a soft copy will be sent to you via email or Messenger.
Reminders:
- Secure your LOA a day before the procedure
- Make sure to stay active for any concerns or query during approval. Your HMO provider will verify information regarding your request procedure.
- Hard copy will be available Monday to Saturday 6am to 6pm at Central Out-Patient Registration.
